Health insurance and GLP-1 cost

How to Get Ozempic or Wegovy Without Insurance in 2026

Last updated September 22, 2026

Paying full retail for a GLP-1 used to mean $1,000+ a month. In 2026 that’s no longer the price most people pay. Both manufacturers now sell direct to patients at a fraction of list price, Medicare has a $50 program, and there are several other legitimate routes worth checking before you give up. Here they are, in the order we’d check them.

1. Manufacturer self-pay programs (start here)

Novo Nordisk (Wegovy, Ozempic) and Eli Lilly (Zepbound, Foundayo) both sell FDA-approved medication directly to cash-paying patients. You still need a valid prescription, but no insurance is required.

Medication Self-pay price per month
Wegovy tablets (NovoCare) $149 (1.5 mg) · $199 (4 mg) · $299 (9 mg & 25 mg)
Wegovy injection (NovoCare) $199 intro for 0.25/0.5 mg (first 2 fills, through Dec. 31, 2026) · $349 for 1–2.4 mg · $399 Wegovy HD 7.2 mg
Zepbound KwikPen (LillyDirect & retail pharmacies) From $299 (2.5 mg starting dose); higher doses cost more
Foundayo (LillyDirect) From $149 (lowest dose)

Prices change often — confirm at checkout on NovoCare or LillyDirect. Ozempic is prescribed for type 2 diabetes; if that’s your diagnosis, check NovoCare’s current Ozempic self-pay price and your plan’s diabetes coverage, which is often better than weight-loss coverage.

2. On Medicare? The $50 GLP-1 Bridge

Since July 1, 2026, eligible Medicare Part D enrollees can get Wegovy, Zepbound KwikPen or Foundayo for $50 a month, and the program now runs through December 31, 2027. Eligibility depends on your BMI and certain health conditions. Full details: Medicare GLP-1 Bridge guide.

3. Patient assistance programs

If you’re uninsured or underinsured with limited income, both manufacturers run patient assistance programs that can provide medication at low or no cost for people who qualify. Rules and covered drugs change, so check eligibility directly with NovoCare and Lilly Cares, or ask your clinic’s social worker or pharmacist to help you apply.

4. Community health centers

Federally qualified health centers (FQHCs) charge for visits on a sliding scale based on income, and many can help you apply for assistance programs. Find one near you at findahealthcenter.hrsa.gov.

5. Clinical trials

Obesity and diabetes trials often provide study medication and monitoring at no cost. Search ClinicalTrials.gov for trials near you — and understand you may receive a placebo.

6. Your employer, HSA or FSA

If your employer plan excludes weight-loss drugs, ask HR whether a coverage rider is available — some insurers sell them to larger employers. Prescription medications are generally eligible HSA/FSA expenses, which effectively discounts self-pay by your tax rate. What to avoid

  • Sellers promising “the same drug” for far less. In March 2026 the FDA sent warning letters to 30 telehealth companies over misleading compounded GLP-1 marketing (Venable LLP). Compounded products are not FDA-approved, and quality varies.
  • “Research use only” peptides and unlicensed online pharmacies. No prescription required is a red flag, not a bargain.
  • Buying from overseas without a prescription. You can’t verify what’s in the pen or how it was stored.

Any legitimate route starts with a licensed prescriber and FDA-approved medication.

The bottom line

For most people paying cash in 2026, the cheapest safe option is a manufacturer self-pay program — often $149–$349 a month instead of $1,000+. If you’re on Medicare, check the $50 Bridge first. If income is the barrier, apply for patient assistance and look up your nearest community health center.
