Headlines promise dramatic transformations. The clinical trials give a more useful picture: meaningful average weight loss, wide variation between people, and results that depend heavily on staying on treatment and building habits that last. Here’s what to realistically expect.

What the major trials found
| Trial | Medication | Average weight loss | Placebo |
|---|---|---|---|
| STEP 1 (68 weeks) | Semaglutide 2.4 mg | 14.9% | 2.4% |
| SURMOUNT-1 (72 weeks) | Tirzepatide 5–15 mg | 15.0%–20.9% | 3.1% |
Sources: STEP 1, NEJM 2021; SURMOUNT-1, NEJM 2022. These are averages from people who also received lifestyle counseling. Some people lose much more, some much less — and your prescriber is the right person to set expectations for you.
How results unfold over time
Weight loss is usually gradual in the first months while the dose is being increased, then steadier, and it tends to level off — a plateau — somewhere after the first year in the trials. A plateau isn’t failure; it’s how the body responds. Tracking non-scale results such as blood pressure, blood sugar, energy and how clothes fit gives a fuller picture.
What happens if you stop
When people in STEP 1 stopped semaglutide, they regained about two-thirds of the weight they had lost within a year, and improvements in heart-health markers largely reversed (Diabetes, Obesity and Metabolism, 2022). That’s why GLP-1s are considered long-term treatments for a chronic condition. Any decision to stop or taper should be made with your prescriber. More: what happens when you stop.
Habits linked to lasting results
- Strength training two to three times a week to protect muscle.
- Protein at every meal — use the protein calculator.
- Consistent sleep and activity — see Lifestyle on GLP-1.
- Staying on your medication without gaps — the supply tracker helps you reorder on time.
The GLP-1 Success Journal gives you a simple weekly structure to track all of this.
